Modeling Hints at a Long-Term Survival Plateau for Myeloma Combination Therapy
MajesTEC-3 Model Projects Near-Normal Survival
Johnson & Johnson's modeling projects 18.5-year life expectancy with Tecvayli-Darzalex versus 4.9 years for standard care. The analysis comes from Phase 3 MajesTEC-3 study showing sustained disease control and survival with Tecvayli (teclistamab) plus daratumumab in patients with relapsed or refractory multiple myeloma who had received 1-3 prior lines of therapy.
In an analysis to be presented in Abstract OA-58, statistical modelling estimated that more than 85% of patients treated with teclistamab plus daratumumab may achieve long-term survival consistent with a functional cure plateau.
What the Combination Is
New model-based analysis further supports the potential of Tecvayli (teclistamab) plus daratumumab to redefine long-term survival expectations in early line relapsed/refractory multiple myeloma. Johnson & Johnson, a worldwide leader in multiple myeloma, announced new data from the Phase 3 MajesTEC-3 study on September 23, 2026.
Teclistamab is a BCMAxCD3 bispecific antibody, daratumumab is an anti-CD38 monoclonal antibody. The combination pairs T-cell redirection with CD38-mediated immunomodulation, building on daratumumab-based regimens that have transformed treatment of multiple myeloma and remain foundational therapy across all stages.
How Modeling Found a Plateau
Johnson & Johnson reported new model-based analyses from the Phase 3 MajesTEC-3 study indicating that teclistamab plus daratumumab may substantially extend survival. The data, drawn from a relative survival mixture cure model (MCM) and a separate post hoc competing-risk analysis, were released on September 23, 2026.
The mixture cure model separates patients into long-term survivors whose mortality returns to general population levels and those who remain at excess risk. For Tecvayli-Darzalex, the model suggests the curve flattens, hinting at a plateau rather than continuous decline.

The 36-Month Data Behind the Model
At a median follow-up of 34.5 months, the combination of teclistamab and daratumumab produced one of the largest improvements ever seen in a major myeloma trial: Progression-free survival at 36 months was 83.4% with teclistamab-daratumumab, compared with 29.7% on the standard combination.
Teclistamab plus daratumumab shows 18-point survival gain at 3 years in multiple myeloma. An 18-percentage-point survival gap at three years is the number that makes the MajesTEC-3 data worth examining closely, in a population with only one to three prior lines.
Why Moving Earlier Matters
The analysis included 646 patients with relapsed multiple myeloma who previously underwent one to three lines of treatment. Thirty-five percent were refractory to their most recent therapy. That gap, in a population with only one to three prior lines, is the strategic argument for moving a BCMA-targeting bispecific earlier in the treatment sequence.
Mayo Clinic-led data shows no improvement in overall survival from longer lenalidomide maintenance beyond two years in standard-risk myeloma, reinforcing that more chemotherapy duration is not the answer. Immunotherapy earlier may be.
Implications for the Myeloma Field
Therapy enables 89% survival rate in myeloma patients with Talvey plus daratumumab combinations showing progression-free survival up to 81.3% versus 51.2% standard. The MajesTEC-3 modeling of 18.5-year life expectancy versus 4.9 years standard care reframes myeloma from incurable chronic disease to potentially curable in a subset.
Risk-adapted quadruplet therapy sustains 5-year survival benefit in high-risk myeloma in the OPTIMUM/MUKnine trial, showing plateau concepts extend to high-risk disease when immunotherapy is intensified.
Cautions and Next Validation
Models are not mature overall survival. The mixture cure model assumes long-term survivors behave like the general population, which needs longer follow-up and real-world validation. Toxicity, infection risk from BCMA bispecifics and daratumumab, and cost of continuous therapy remain issues before a plateau can be declared clinically.
