Novo's Next Weight-Loss Bet Is Already Beating Its Own Blockbuster
Novo Nordisk’s experimental combination therapy CagriSema has delivered stronger weight loss and glycaemic results than the company’s own blockbuster Wegovy (semaglutide) in a late-stage trial of people living with both obesity and type 2 diabetes. In the Phase 3 study, CagriSema produced greater reductions in body weight and comparable or better improvements in A1C than semaglutide alone, reinforcing its position as the leading next-generation injectable in Novo’s obesity pipeline. The data arrive as the company prepares for a potential early-2027 launch window and seeks to defend its franchise against intensifying competition, particularly from Eli Lilly’s tirzepatide (Zepbound).
What CagriSema Is
CagriSema is a once-weekly injection that combines cagrilintide, a long-acting amylin analogue, with semaglutide, the GLP-1 receptor agonist that powers Wegovy and Ozempic. Amylin works alongside GLP-1 to enhance satiety, slow gastric emptying and reduce food intake. By engaging both pathways, the combination aims to produce deeper and more durable weight loss than GLP-1 monotherapy while retaining strong effects on blood sugar.
Phase 3 Results Versus Wegovy
In patients with type 2 diabetes and obesity, CagriSema achieved greater weight reduction than Wegovy across both the efficacy estimand (patients who stayed on assigned treatment) and the treatment-regimen estimand (closer to real-world adherence). On the completer analysis, weight loss reached approximately 14% with CagriSema versus about 10% with Wegovy; blood-sugar reductions were also numerically favourable. In a separate late-stage obesity trial, a 1.0 mg dose of CagriSema produced around 21% weight loss versus placebo. Novo has also reported that, in one head-to-head comparison at selected doses, CagriSema outperformed a lower dose of tirzepatide on weight loss in diabetes patients.
Strategic Importance for Novo
Wegovy transformed the obesity market but faces eventual patent pressure and fierce competition from dual and triple agonists. CagriSema is positioned as the natural successor: same weekly injection format, familiar semaglutide component, and an added amylin mechanism intended to raise the efficacy ceiling. Positive Phase 3 data against the current standard help de-risk regulatory filings and give physicians a clear upgrade path for patients who need more weight loss or better glycaemic control than they achieve on Wegovy alone.

The Competitive Reality Check
Despite beating its own drug, CagriSema still faces a high bar set by Lilly’s tirzepatide, which has posted larger average weight-loss figures in several trials and has taken the lead in US obesity sales. Earlier head-to-head data were mixed, and analysts continue to debate whether CagriSema can fully close the gap with Zepbound on pure weight-loss magnitude. Tolerability, manufacturing scale, pricing and real-world persistence will all influence the commercial outcome once the drug reaches the market.
Regulatory and Launch Path
Novo has filed or is preparing filings for CagriSema and has outlined a multi-year rollout of next-generation obesity injectables, with CagriSema targeted for launch as early as 2027 in key markets. Additional data on cardiovascular outcomes, longer-term maintenance and specific subpopulations will further shape labelling and uptake. Parallel work on oral and other injectable candidates continues, but CagriSema remains the most advanced combination in the company’s near-term portfolio.
Outlook
By outperforming Wegovy on both weight and glycaemic endpoints in patients with diabetes and obesity, CagriSema has cleared an important internal benchmark and strengthened Novo Nordisk’s claim to a best-in-class next-generation injectable. The bigger test will be how the drug performs against the current market leader and whether the added amylin component translates into meaningful differentiation in everyday practice. For now, the data confirm that Novo’s next weight-loss bet is already beating the blockbuster it is meant to succeed—an encouraging signal as the company races to stay ahead in the most competitive metabolic market in decades.
